When you or a loved one is facing a serious illness and curative treatment is no longer the goal, hospice care shifts the focus to comfort, dignity, and quality of life for the days, weeks, or months ahead.

Hospice is specialized end-of-life care for individuals with a terminal illness who are certified by a physician to have a life expectancy of six months or less if the illness runs its normal course.

Through expert pain and symptom management, emotional support, and compassionate guidance for families, hospice provides steady care centered on peace, presence, and the moments that matter most.

Meet Your Hospice Team

At T.O.N.E. We at Hospice & Palliative Care, Inc. are partners in your care, not just caregivers. At the center of our approach is our Interdisciplinary Team (IDT), with you and your loved ones as valued members. Together, we support your physical, emotional, and spiritual needs with compassion and clinical expertise.

We’re your team—ready to listen, adapt, and care for what matters most to you. If you have questions, just reach out anytime.

Your Personal Physician

You can continue seeing the doctor you know and trust. They’ll collaborate with us on your treatment, medications, and overall plan of care—always with their approval.

Our dedicated Medical Director oversees your care closely, offering consultations, home visits, and coordination with your personal physician whenever needed. We’re here for you every step.

Your RN is your care coordinator, visiting regularly to assess your condition, manage pain and symptoms, and ensure your comfort. They bring expertise and a gentle touch to every visit.

Our social workers support your whole family emotionally during this challenging time. They help with daily concerns, connect you to community resources, assist with placements or funeral planning, guide you through advance directives, and offer specialized counseling for children

Chaplain

Our chaplains provide spiritual care tailored to your beliefs—non-denominational and respectful of all faiths. They support you and your family, and connect you with your own clergy if desired.

Our aides offer hands-on help with bathing, dressing, meals, and daily activities, giving you dignity and relief from routine tasks.

Our volunteers bring warmth through companionship, errands, light housekeeping, or respite for caregivers.

From admission through up to two years of grief support, our bereavement counselors walk with your family through loss with understanding and resources.

Medicare Hospice Benefit

When will Medicare Pay for Hospice Benefits?

Medicare will pay for hospice benefits when the following conditions are met:

  • The patient is eligible for Medicare Part A  
  • The patients attending physician and the TONE Hospice and Palliative Care Medical Director agree that the patient meet hospice eligibility criteria  
  • The patient/or designee signs Medicare Hospice Election Form to choose the Medicare Hospice Benefit  

Medicare will continue for covered benefits for any health problems that are not related to the hospice diagnosis. 

The Medicare Hospice Benefit typically covers the following services related to your terminal diagnosis:

  • Physician services  
  • Nursing care  
  • Medical equipment such as wheelchairs, walkers, and specialized beds  
  • Medical supplies such as bandages and catheters  
  • Medications for symptom control or pain relief related to your hospice diagnosis 
  • Home health aide and homemaker services  
  • Physical and occupational therapy  
  • Speech therapy  
  • Social worker services  
  • Dietary counseling  
  • Grief and loss counseling for you and your family 
  • Short-term inpatient care for pain and symptom management  
  • Short-term respite care   
  • Any other Medicare-covered services to manage your pain and other symptoms as recommended by your T.O.N.E. Hospice and Palliative Care team

The Medicare Hospice Benefit does not cover:

  • Curative treatment  
  • Medications not related to relieving pain/symptoms not related to the hospice diagnosis  
  • Room and board in a nursing home or hospice facility for anything other than respite care or short-term inpatient care for pain/symptom management  
  • Care in an emergency room, hospital, or ambulance transportation unless arranged by your TONE Hospice and Palliative Care team, or if the care is unrelated to the hospice diagnosis
